MyChart – Our Patient Portal
MyChart is a secure, online connection to your medical information all from the convenience of your tablet, smartphone, or computer. It removes barriers that keep patients from getting what they need from their healthcare provider. It’s all about access.
With the MyChart website or app you can:

Send a request when you run out of medication refills.
Download the MyChart App
Search “MyChart” in the App Store or Google Play on your smartphone or tablet. Be sure to select “SLHO Health System” as your organization before you login. Using MyChart is easy, and makes navigating the health system more efficient.

Access your test results
No more waiting for a phone call or letter – view your results and your doctor’s comments within days.

Manage your appointments
Schedule your next appointment, or view details of your past and upcoming appointments.

Pay bills online
Access, review and pay your bills using your credit or debit card.
How do I sign up for MyChart?
At your next physician appointment, ask your care team for information about SLHO MyChart. They can give you an overview of the digital tool, instructions for signing up, and the unique access code you’ll use to create an account for yourself and those you have proxy for, such as your children. If you need access before your next appointment, call (404) 616-5551.
Once you have the instructions and your unique access code, go to to log on to SLHO MyChart. Remember, since SLHO MyChart provides secure access to your personal medical health records, you should not share your password with anyone you do not want accessing your private health information.
If you have a medical emergency, please call 911. Do not rely on MyChart to communicate emergency medical needs. MyChart is not intended as a substitute for regular, in-person patient/physician communications that should occur during your office visits.
